Use this check list to review the steps required to create Training Event Feedback within Self-Service and report on the data within Cintra iQ.

Define the feedback form for each training course category or for all categories. | A feedback form can be defined for each training course category. Or, you can use the same feedback form across all categories. |
Decide what the questions are for each form. | You can create as many or as few questions as needed to obtain the information you want. |
Define the Feedback questions in the iQ database. | Assign the question group to the training category via the TRAINING_FEEDBACK_QUESTION_GROUP table in the iQ database. |
Ensure Learning & Development is licensed in Self-Service. | For more information about Licensing Learning & Development, see How Do I set up Learning and Development. |
Set up the Roles to enable feedback forms to be used within Self-Service. | Edit the Self-Service user's and approver's role to include the Training Feedback functionality. |
Ensure the user has attended and completed the course. | In order for the training event to be recognised in Self-Service, the course completion and the attendance date must be set in the past. |
